Orthodontic Treatment with Distraction Osteogenesis for a Skeletal Class III Patient with Unilateral Cleft Lip and Palate: A Case Report

Abstract
Maxillary deficiency is a common feature in patients with repaired cleft lip and palate. Orthognathic surgery has been a conventional approach in adult patients with cleft-related severe maxillary deficiency. However, for growing patients with a severe maxillary deficiency, distraction osteogenesis has many advantages, such as larger amount of maxillary advancement, higher stability, better soft tissue adaptation, lower possibilities of velopharyngeal problems when compared to the orthognathic surgery. In this case report, we present on the use of a rigid external distraction device for maxillary advancement in a 14-year-old male patient with a unilateral cleft lip and palate and severe mid-facial deficiency. After distraction osteogenesis, orthodontic and prosthodontic treatments were performed for functional and esthetic improvement of occlusion and facial profile. Maxillary distraction osteogenesis and consecutive orthodontic treatment provided effective treatment for adolescent patient with cleft lip and palate, and which maintained well after 1.5-year retention.
